Aqua Vida Membership Program Disclosures

1. General Terms and Conditions

• Membership Duration: The membership term varies depending on the package selected. After the required term, your membership will automatically renew for successive terms unless canceled in writing at least 30 days before the renewal date. This can be done via email or letter.
• Fees and Payments: Membership fees are determined by the specific package selected. Monthly payments will be automatically charged to the credit card on file. Should you elect to pay the full membership fee upfront, you may qualify for an additional discount. Any services or treatments not included in the membership package will incur separate charges.
• Commitment Period: Membership packages require a minimum commitment period. Please review your package’s requirements.

2. Service Limitations

• Treatment Availability: Membership includes the specific treatments and session limits defined by the chosen package. All membership services are subject to availability. Please make appointments ahead of time. Walk-ins are welcome but not guaranteed.
• Service Exclusions: Membership does not include any treatments or services outside of those specified in the selected package.

3. Cancellation and Refund Policy

• Early Termination: If you wish to terminate your membership before the end of the term, the cancellation fee will be equivalent to the total discount received during the membership period for any treatments and 25% of the remaining cost of the membership term. No additional refunds will be issued for unused services upon early termination. Cancellation must be made in writing 30 days before the renewal date. This can be done via email or letter.

4. Medical Disclaimers

• Health Conditions: A good faith exam is required for all new clients receiving medical treatments. This exam will be provided on-site by our qualified providers to assess your suitability for the treatments. You must disclose any medical conditions, medications, or allergies that may affect your ability to safely receive these treatments.

5. Appointment and Rescheduling Policies

• No-Show and Late Cancellation Fees: A no-show or cancellation with less than 24 hours’ notice will incur a $50 fee. Repeated no-shows may result in suspension or termination of membership. VIP members are excused from this policy and will not be charged late or no show fees. 
• Rescheduling: Appointments can be rescheduled up to 24 hours before the scheduled time without penalty. Rescheduling with less than 24 hours’ notice will incur a $25 fee.

6. Membership Benefits

• Perks and Discounts: Aqua Vida reserves the right to modify membership benefits. Any changes to benefits will not affect the perks available to you until the expiration of your current membership term. Membership benefits cannot be combined with other promotions, but you may hold more than one membership simultaneously.

7. Privacy and Confidentiality

• Data Protection: Personal and medical information collected during your membership will be used solely for providing and managing aesthetics and wellness services and will be protected according to applicable privacy laws. Your information will not be shared with third parties without your consent.

8. Liability and Waivers

• Assumption of Risk: By participating in the membership program, you acknowledge the inherent risks associated with aesthetics and wellness treatments and agree not to hold Aqua Vida liable for any adverse effects or injuries resulting from these treatments.
• Indemnification: You agree to indemnify and hold harmless Aqua Vida, its employees, and affiliates from any claims, damages, or liabilities arising from your use of the membership services.

9. Membership Changes and Cancellations

• Modifications: Aqua Vida reserves the right to modify membership terms and benefits. Your current perks will remain in effect until the expiration of your membership term.
• Cancellation Rights: Aqua Vida reserves the right to cancel any membership for any reason deemed fit. Failure to follow guidance from our medical providers will result in immediate cancellation with no refund issued.
